- Date of Birth certificate (Secondary School/Matric Certificate- 01 leaf.
- Domicile certificate — 01 leaf
- BDS Degree Certificate.
- PG Certificate, along with Registration certificate, from competent authority- upto 04 leaves.
- Two years experience certificate from the teaching institutions recognized by the Dental Council of India, as per format.
Edit Option: Candidates who have successfully submitted the online application form along with requisite fee will be allowed to edit some of the fields in their submitted online application form within three days after the cut-off date i.e. from 30.6.2023 to 02.7.2023.
