Applications are invited for filling up vacant posts of Specialists at ESIC Model Hospital, Bari-Brahmana, Jammu as per the details given below:
Eligibility Criteria:
General Instructions:
- Candidates fulfilling the educational qualification & other eligibility conditions should submit their applications with one set of self-attested copies of the relevant documents (as listed under CHECKLIST) along with application form as per Annexure ‘A’. The candidates have to bring all the original documents as per checklist for verification.
- Candidates should report in the Conference Hall situated at First Floor of ESIC Model Hospital, Bari-Brahmana, Jammu, at 10.00 A.M. on the stipulated date.
- Documents will be accepted for scrutiny strictly till 11.00 A.M. Only eligible candidates will be called for interview.
- DETAILS OF APPLICATION FEE: Candidates have to deposit application Fee by Demand Draft (Non-Refundable) for Rs. 250/- drawn in favor of ‘ESI Fund A/c No. 1[1] payable at SBI, Jammu.
- No Application fee in case of SC/ST/PWD and female candidates. The candidates working in Govt. Service should produce NOC from their department at the time of interview.
- The Medical Superintendent reserves the right to fill all or none vacancies, to alter the date or cancel the interview without any prior notice.
- The number of vacancies may be changed without any previous notice.
- No TA/DA will be paid for interview.
- In case the eligible candidates are available for both Full-time and Part-time Specialist post for any one Specialty then preference will be given to Full Time Specialist post.
Terms & Conditions:
For Full Time Contractual Specialists
- Specialists shall be recruited for a period of 3 years with annual evaluation and term renewed every year based on satisfactory performance.
- Private practice will not be allowed.
- Available on Call 24×7.
- The Specialists shall be responsible for all cases undergoing treatment under his supervision in his/her discipline.
- He / She will be entitled for the leave @ 04 days for each month of work on prorata basis in a calendar year. In the event, the Specialist prefers not to avail the leave, the same may be encashed to a maximum limit of 30 days/year.
- The selected specialist must have a professional indemnity policy which should cover the period as desired by ESIC in the bid. The policy is meant to cover professional liability falling on them as a result of error and omissions committed by them while rendering professional services. The minimum sum assured per annum should be Rs. 30 Lacs for Anesthesiologists, Rs. 20 Lacs for ENT Surgeons and Rs. 10 Lacs for Radiologists, Dermatologists and Pediatricians. A copy of the Insurance & Premium and receipt must be submitted within 7 days from the date of engagement.
- The contractual engagement may be terminated/discontinued on either side giving one- month prior notice to this effect without assigning reasons.
For Part Time Contractual Specialists
- Period of contract one year from the date of joining.
- Minimum working hours will be 16 per week as per directions of MS.
- He/She will not be entitled for any kind of leave or benefits.
- Available on call 24×7 as he will be drawing emergency call charges.
- The Specialists shall be responsible for all cases undergoing treatment under his supervision in his/her discipline.
- The selected specialist must have a professional indemnity policy which should cover the period as desired by ESIC in the bid. The policy is meant to cover professional liability falling on them as a result of error and omissions committed by them while rendering professional services. The minimum sum assured per annum should be Rs. 30 Lacs for Anesthesiologists, Rs. 20 Lacs for ENT Surgeons and Rs. 10 Lacs for Radiologists, Dermatologists and Pediatricians. CHECKLIST
(DOCUMENTS TO BE SUBMITTED WITH APPLICATION FAILING WHICH
APPLICATION SHALL BE SUMARRILY REJECTED); –
- Matriculation certificate as proof of age.
- Permanent Registration with MCI / State Medical Council.
- MD / DIPLOMA / MBBS Degree.
- Attempt Certificates and Marks Sheet of MD / DIPLOMA
- Experience Certificate.
- NOC from present employer, if applicable.
- Caste Certificate (if applicable) in the prescribed format of GO1 issued on or after 01/04/2017 by the Competent Authority of the State /GOI.
- OBC Certificate.
- Two recent passport size photographs.
- Self-attested copy of Aadhaar and PAN card.
- Annexure ‘A’, ‘B’ & ‘C’
